2022 Remembrance Tree Ceremony

About the Remembrance Tree 

For decades, the Remembrance Tree Ceremony has been a chance for the community to remember loved ones during the holiday season. Join us on Sunday, December 4, at 2 PM, as we remember those we miss when families gather for the holidays.  

As always, the ceremony will include special readings, music, a memorial photo slideshow, and the reading of your loved ones’ names. Anyone who misses a loved one during the holidays is welcome to participate, regardless of whether your loved one was cared for by our hospice. 

Due to the ongoing concerns surrounding COVID-19, we will  again hold the 2022 Remembrance Tree Ceremony as a live, virtual event via Facebook Live on the Remembrance Tree’s Facebook Event Page, 

To Request Your Memorial Ornament 

Please use the donation form below to request your memorial ornament. You may request ornaments online through November 30. After November 30, please contact us at [email protected] to make your request. 

When you request an ornament, it will be personalized with your loved one's name. When your loved one’s name is read during the Remembrance Tree Ceremony, our staff will place your ornament on a Remembrance Tree. 

Due to HIPAA regulations, we can only read names that families request us to read. We are not able to include any names automatically. 

We ask that you make a donation to request an ornament, but the donation can be of any size. All proceeds support the cost of the 2022 ornaments and any excess will fund hospice, palliative, and bereavement care for patients and families, regardless of insurance status or ability to pay.  

Many families' incomes have been adversely affected by COVID-19. If you wish to request an ornament but making a donation is a barrier to you, please contact us at 859-986-1500 during normal business hours. 

Your donation is tax-deductible to the fullest extent of the law. 

To Participate in the Optional Memorial Slideshow: Deadline Nov. 30 

Participation in the memorial photo slideshow is optional. To include a photo of your loved one in the slideshow, please submit a digital copy of the photo, along with their full name, to [email protected] no later than Nov. 30. 

How to Participate in the Facebook Live Event 

You do not need a Facebook account to view the ceremony. However, if you wish to interact by using the comments section to type your loved one's name as it is read or to share a favorite memory, you will need a Facebook account. 

How to Claim Your Memorial Ornament After the Ceremony 

You will have an opportunity to claim your ornament at these times and locations. If you need to make other arrangements, please contact us at [email protected] or 859-986-1500. We apologize, but we are unable to ship ornaments.
