Pre-Registration Form

Our Grief Support Series in Estill County is on Wednesdays at 10 a.m. starting May 29, 2024, and ending with a shadowbox workshop on July 3. You will receive additional details by email once pre-registration is complete.

This Grief Support Series is for individuals age 18 and up.

To pre-register, complete the form below. Denise McKinney, our Grief & Loss Support Coordinator, will contact you to complete registration. If you prefer to pre-register by phone or have questions, contact Denise at grief@hospicecp.org or 859-986-1500.

We also offer a Grief Support Series in other locations throughout the year, a weekly Grief Support Gathering at our Compassionate Care Center, an annual remembrance ceremony, and one-on-one support services. Please visit our website for additional information on these services.

Note: Although the pre-registration forms asks if your loved one was cared for by our organization, the Grief Support Series is open to the community at large.
