Pre-Registration Form

Please use this form to pre-register for any of our Grief & Loss Support services. After you complete pre-registration, Denise McKinney, our Grief & Loss Support Coordinator, will contact you to complete the process.

If you are specifically interested in group offerings, you may wish to visit our website. We currently host multiple groups in several locations throughout the year. Our website has details and pre-registration links for all current groups.

NOTE: We can provide grief and loss support in these Kentucky counties only: Estill, Jackson, Lee, Madison, Owsley, and Rockcastle.
