Pre-Registration Form

The Hospice Care Plus Journaling Through Grief series begins Jan. 14 and ends June 24. The group meets every second and fourth Tuesday at 5:30 p.m. Meetings are held at the Madison County Public Library in Berea. You will receive additional details once pre-registration is complete.

We ask that you pre-register before attending the session for the first time. To pre-register, complete the form below. Denise McKinney, our Grief Support Coordinator, will contact you to complete registration. If you prefer to pre-register by phone or have questions, contact Denise at grief@hospicecp.org or 859-986-1500.

We also offer a weekly Grief Support Gathering at our Compassionate Care Center, Grief Support Series in multiple locations throughout the year, an annual remembrance ceremony, and one-on-one support services. Please visit our website for additional information on these services.

Note: Although the pre-registration forms asks if your loved one was cared for by our organization, the Journaling Through Grief series is open to the community at large.
