New Volunteer Training Sign-Up Form

Enter your information to sign up for the new volunteer training on Saturday, June 8, from 9:30 a.m. to 4:30 p.m., at the Madison County Public Library in Berea. This is an in-person training.

Once you complete the form below, you are officially registered for the training. If you plan to bring guests, please ask them to sign up separately.

If you wish to sign up for self-paced, online training instead of this in-person training, please answer "yes" to the last question.

After signing up, complete and return your Volunteer Application. Watch for details in your confirmation email (please check your spam or junk folder if you don't see it within 12 hours).

We will ask you to complete a few online training modules prior to the training. Mariah Smith, Volunteer Coordinator, will get in touch with you individually to help you with that process. Completing some of the modules online that regulatory bodies require us to provide, such as infection control and workplace safety, allows us to focus the in-person training on more meaningful aspects of hospice care, palliative care, and bereavement.

On training day, we will provide lunch. There will be plenty of breaks, snacks, drinks, and many opportunities to ask questions.
